We agree that the pelvic exenteration procedure is a large complex procedure with
a large perioperative morbidity. In our cohort 34.4% of the patients experienced major
complications. Many could be handled radiologically or medically. It does not, however,
change the fact that patients should be selected carefully for this procedure.

- PreviewPelvic exenteration represents a maximally radical procedure for the management of pelvic malignancies. Because the risk of morbidity following pelvic exenteration is very high, this procedure in my opinion should be reserved for patients being management with a curative intension. The benefit of this extensive and partly disfiguring surgery, i.e. potential cure of a malignant disease must be weighed against the risk of complications.
